In this role, you will have the opportunity to make a positive impact on the lives of our members by providing comprehensive case management services. Provide comprehensive case management services to Aetna members in Woodford and McLean Counties. Utilize strong clinical knowledge to assess and develop individualized care plans for members. Communicate effectively with members, healthcare providers, and other team members to ensure coordinated and efficient care. Conduct ongoing evaluations and adjustments to care plans based on member progress and changing needs. Collaborate with healthcare providers to ensure appropriate and timely treatment for members. Coordinate and facilitate access to necessary medical resources and services for members. Advocate for the needs and rights of members to ensure they receive high-quality and compassionate care. Maintain accurate and timely documentation of all interactions and interventions with members. Adhere to all relevant policies, procedures, and regulations related to case management and healthcare. Continuously evaluate and improve case management processes to ensure optimal outcomes for members. Participate in training and development opportunities to enhance clinical knowledge and skills. Maintain confidentiality and privacy of member information at all times. Represent Aetna in a professional and ethical manner at all times. Maintain a positive and supportive attitude towards members, colleagues, and the company. Stay updated on industry trends and best practices in case management to ensure high-quality services for members.
