Occupational Therapist (OTR/L) – Mobile Home Health Therapy Provider

Responsibilities
- Perform in-depth in-home evaluations focused on function, safety, and independence.
- Design customized treatment plans based on real-life environments.
- Address ADLs, mobility, cognitive function, and home safety barriers.
- Recommend and implement adaptive equipment and environmental modifications.
- Educate patients and caregivers to drive lasting outcomes.
- Document and collaborate with an interdisciplinary team.
Requirements
- Active Minnesota OTR/L license.
- Strong clinical judgment and problem-solving ability.
- Confidence working independently in a field-based setting.
- Passion for delivering outcome-driven, patient-centered care.
- Reliable transportation for home visits.
