Overview

We are seeking a dedicated and experienced Registered Dietitian and Diabetes Educator to join our healthcare team. This role involves providing expert nutritional guidance, patient education, and care planning for diverse patient populations, including those in long-term care, acute care, geriatrics, and hospital settings. The ideal candidate will possess strong knowledge of physiology, nutrition, and dietetics, with a focus on managing chronic conditions such as diabetes and weight management. This position offers an opportunity to make a meaningful impact on patient health outcomes through personalized care and education. Duties

  • Develop and implement individualized nutrition care plans based on comprehensive patient assessments.
  • Provide diabetes education, including blood glucose management, insulin administration, and lifestyle modifications.
  • Utilize motivational interviewing startegies for encouraging and motivating patients to elicit behavior change
  • Conduct diagnostic evaluations to determine nutritional needs and identify potential health risks.
  • Offer health coaching to support patients in achieving weight loss goals and lifestyle changes.
  • Utilize Electronic Medical Records (EMR) systems to document assessments, care plans, and progress notes accurately.
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Educate patients on nutrition principles chronic disease control.
  • Monitor patient progress and adjust care plans as needed to optimize health outcomes.

Qualifications

  • Registered Dietitian (RD) certification required; licensure or certification as a Diabetes Educator is preferred.
  • Strong knowledge of physiology, nutrition science, dietetics practices, and diagnostic evaluation techniques.
  • Experience with health coaching, motivational interviwing, patient assessment, and developing personalized care plans.

Job Types: Full-time, Part-time Pay: $33.00 per hour Expected hours: No less than 20.0 per week Application Question(s):
