Role Overview Join Divine Mercy Behavioral Health Center as a Licensed Clinical Social Worker (LCSW) in a remote capacity. In this vital role, you will provide compassionate care and support to patients within our hospice center, ensuring their emotional and psychological needs are met.

Key Responsibilities

• Conduct assessments and develop care plans tailored to individual patient needs.
• Provide counseling and support to patients and their families during challenging times.
• Collaborate with a multidisciplinary team to ensure holistic patient care.
• Document patient progress and adjust treatment plans as necessary.
• Participate in staff meetings and continuing education to enhance service delivery.

Required Qualifications

• Master’s degree in Social Work (MSW) from an accredited institution.
• Current LCSW licensure in the state of Illinois.
• Experience in hospice, palliative care, or related fields is preferred.
• Strong interpersonal skills and the ability to empower patients and families.
