Intake Specialist / Long-Term Care Liaison (Bilingual Spanish) 100% REMOTE, BUT MUST LIVE IN TRISTATE AREA. About the Opportunity A growing, licensed home care provider serving the New York City metropolitan area is seeking a Bilingual Spanish Intake Specialist / Long-Term Care Liaison to join its team. This position is ideal for someone who is passionate about helping seniors access quality home care services and enjoys serving as a trusted resource for patients and their families throughout the enrollment process. The successful candidate will process referrals, coordinate with healthcare providers and payors, and guide prospective clients through the steps necessary to establish home care services.
Responsibilities
- Educate prospective clients and their families regarding the requirements and process for initiating home care services.
- Serve as a liaison between clients, family members, nursing facilities, physician offices, hospitals, and insurance providers.
- Coordinate assessments and assist with the enrollment process while ensuring clients remain informed throughout each stage.
- Process and manage daily referrals received from the marketing and business development team.
- Maintain accurate documentation and update daily reports using the organization's care management systems.
- Provide exceptional customer service and ongoing support to prospective clients and referral sources.
Qualifications
- Minimum of 1 year of experience as an Intake Specialist, Long-Term Care Liaison, or in a similar home care intake role.
- Bilingual in Spanish and English required.
- Experience working with seniors, home care, managed care, or long-term care services preferred.
