Job Description We are seeking a dedicated and compassionate Nurse Case Manager to join our team. The ideal candidate will be responsible for coordinating patient care, ensuring that individuals receive the necessary services and support throughout their treatment journey. This role requires a strong understanding of case management principles, diagnostic evaluation, and the ability to work within various healthcare settings, including hospitals and clinics. *Responsibilities* Facilitate communication between patients, families, and medical staff to promote understanding of treatment options and resources available. Monitor patient progress and adjust care plans as necessary to meet evolving needs. Ensure compliance with HIPAA regulations while handling sensitive patient information. Advocate for patients by connecting them with community resources and support services. Maintain accurate documentation of case management activities. *Qualifications* Bachelor’s degree in nursing, social work, or a related field. CCM Certification preferred but not required. Case management: 2 years (Required) Workers' compensation: 2 years (Required) Clinical Experience: 2 years (Required) Familiarity with Workers' Compensation protocols and procedures is a plus. Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations. Strong organizational skills and attention to detail in managing multiple cases simultaneously. Ability to work collaboratively within a multidisciplinary team while demonstrating empathy and professionalism. ***Generous benefits package after 60 days.
