About the position The Health Outcomes Pharmacist is a hybrid role responsible for driving value creation, cost control, and fiscal targets within the organization. This position involves engaging with primary care providers (PCPs) and facilities to enhance their participation in value-based reimbursement programs. The pharmacist will leverage their expertise in clinical pharmacy and medication management to develop strategies that improve quality, cost, and utilization metrics, ensuring that the organization meets its ROI targets. The role requires collaboration with various stakeholders and the ability to analyze data to identify opportunities for improvement in healthcare delivery. Responsibilities • Serve as the Clinical Pharmacy Subject Matter Expert (SME) and support the broader team in pharmacy-related report interpretation. , • Provide consultative workflow transformation and training services to PCPs and facilities enrolled in value-based reimbursement programs. , • Assess data for the highest ROI opportunities in PCP performance and implement strategic plans to meet identified program targets. , • Function as the organization's representative SME in value-based reimbursement programs, pharmacy, and data analysis. , • Analyze and interpret PCP and facility value-based reimbursement reports and develop strategic plans to meet program targets. , • Independently manage PCP and facility caseloads, projects, and deliverables for individualized strategic plans. , • Collaborate with interdepartmental teams to provide comprehensive support to PCPs and facilities for practice transformation.
