Responsibilities • Provide direct support to patients in their homes, ensuring their comfort and safety. • Assist with daily living activities and personal care as needed. • Communicate effectively with patients and their families to address concerns and provide updates. • Maintain accurate records of patient interactions and care provided. Requirements • Experience in home health is preferred. • Strong interpersonal skills with a focus on customer service. • Knowledge of resident rights and advocacy for patient needs. • Basic first aid certification is a plus. • Ability to work effectively with individuals with developmental disabilities or memory care needs.
